I am Reina Solomon, Office staff at ERD Department. I would like to request for financial assistance for my Temporomandibular Joint (TMJ) disorder. It is a pain in the jaw joint that can be caused by a different medical problems. The TMJ Specialist advise me to wear a dental appliance called splint to be able to correct my jaw alignment and it cost Php 40,000.00.

As you all know, my current income is just enough to cover basic necessities. In this case, may I request for a financial assistance, any amount is very much appreciated. I am planning to raise the above amount for the soonest possible time in order for me to back to work.
