Decline Letter for Covid Vaccine

July 7, 2021

To whom it may concern:

I am choosing to decline COVID-19 vaccination right now. I am truly aware the effects of my decision but I do believe that this is wise choice since I have allergies to vaccine contents.

Based on my knowledge the vaccine is on trial stage so there is no guarantee that it will be safe to use especially for those who have weakened immune system.

I hope for your understanding regarding this matter.

Sincerely yours,

Patricia A. Vienne

Decline Letter – Appointment

October 10, 2001

Maricon E. Navarro
Blk. 5 Lot 8 Talahis Subd.
Orani, Bataan

Dear Ms Navarro:

I consider my appointment as Chairman of the Social Committee of the Chicago Club of the Philippines a very great honor.  The success of the club rests largely upon this Committee.  Nothing would please me more than to serve with the other members of this group.

However, business conditions are so critical at the present time that it is just impossible for me to accept any other responsibilities or assignments outside of my company.  I know you understand that only a matter of grave personal concern prevents me from accepting this appointment.

Regretfully your,

John E. Carlos

Thank you for inviting me as one of the lecturers to discuss on “Protective Coating” in your seminar-workshop on July 12.

I regret to decline this opportunity because of an unavoidable prior commitment.  Nevertheless, I attached a summary of my lecture which could be distributed to the participants as supplementary information.

I hope I could be a service to your company some other time.
