Dear Mr. Dinglasan

I would like to inform you that you will given a chance to prove yourself once again since you have a poor performance of your duties based on your latest evaluation report. This will be grounds for termination of employment .

However, the company would like to give you one last opportunity to prove your value to our company and you will be placing to a six months probationary period effective immediately. And if you failed to improve yourself, we have no choice but to terminate your employment without prior notice.

This probationary period will provide you an opportunity to prove to us that you deserve a second chance. We hope that you will use this opportunity to your advantage.
