I would to request for assessment of my daughter, Althea Samatha R. Guevarra, if she is eligible for Special Education Services. She is 4 years and presently attends school.
I want to assess my daughter in the area of speech, academics and behavior. For the past few weeks, I observed that she is delayed in speech, poor writing skills, and there is no focus. Also, she displays repetitive behavior and no eye contact.
I am looking forward of meeting your assessment team regarding my daughter’s condition. I believe that early intervention is very crucial for my child’s development.
