I, Harlene Bautista authorize Herbert Bautista and Hazel Bautista, my two children to join on educational tour to Palawan on August 21, 2005, aboard airline Philippine Airways at the Subic Airport, and they will accompanied by their uncle who Is an adult to ensure their safety.
