I authorize my cousin, Ms. Rhodora Escudero to perform any acts that are necessary to ensure the health of my daughter, Nicole L. Santos while I am away and she is under her care. She also acts as my daughter’s guardian.
Ms. Escudero will also take care of any doctor or hospital visits, vaccines and medicines of my daughter. If there is any problem with my daughter, just kindly notified Ms. Escudero.
