I, the undersigned name is writing to you this letter as notification that I am allowing the bearer of this letter, Luciana D. Santiago, my cousin to claim my GSIS ID in your office.
I am presently in Vigan to attend personal matters that is why I won’t be able to claim my GSIS ID personally. My cousin bring my employment ID as valid proof and for your reference. I also giving Ms. Santiago a right to sign any papers related to the claiming of my GSID Card.
