Dear Ms. Pambato:

Good day!

This is to inform that may car has been damaged and I would like to claim the insurance amount for it.

Last October 12, 2005 at 11:00 pm, I was driving a car on Acacia St. Sampaloc, Manila and suddenly involved a collision with a car. My car has been damaged such the headlights are broken and the bumper has come out. I immediately took pictures of the damaged portions as a proof to support my argument. The estimated cost of repairs is approximately Php25,000.00.

I already attached my insurance paper and it says that I am eligible for an insurance claim. I have attached the copy of the estimates as well as the the picture of damages.

I hope for your response the earliest possible time.

I am writing to inform you about my insurance claim for my car. My policy number is 015214-5204. Your representative already examines and filed a report regarding my insurance claim.

Here are the little details of my car accident:

The body of my car got smashed because a tricycle hit my car from behind at the parking area at the grocery store. I call your customer care and gave the details and found out that I am eligible for a claim. Your representative told me that they will call me soon regarding this matter and will get the feedback at the early possible time.

I didn’t get any words from your company yet in spite of my reminders for last fifteen days. Please kindly look into this matter and I am expecting a positive response as soon as possible.

I already reported it to the police then and there but so far it has bit been traced at all. It was seven days since my car was lost.

Please let me know the rules under which I may get the claim from your company for the loss.
