I am writing this letter to apply for a salary loan of Php 35,000.00 as I am dire need of money for my kidney stone operation.
My doctor, Dr. Erlina R. Sampang, told me that I need to be operated as soon as possible to prevent any further complication. I enclose here a detailed estimate by the hospital staff. Once I receive the loan, I will schedule myself for my operation.
I hope you can grant my request so that I can get myself operated the soonest possible time. I will pay my salary loan in installment basis which can be deducted from my salary on monthly basis.
