It is with regret that I must inform you that I will no longer be able to serve as your physical therapist. After careful consideration, I have decided to terminate my service based on your history of failing to keep your schedule appointments. I will continue to provide my therapy services to you through October 2, 2005 but you will need to seek other services from a new therapy provider after the said date.

During the last 2 months, you have scheduled and failed to show up for 4 appointments. I understand that sometimes it it necessary to cancel therapy due to unexpected event or unavoidable reason but I cannot tolerate missed appointments on a regular basis.

I suggest that find a therapist near in your place. You can get the copy of your medical records in our office at your convenient time.

  7. Firewall , storage and slab garage and canopies are all painted of plexi bond water proofing and roof shield paint
  8. Jetmatic and water tank are owners responsibilities
  9. Perimeter fence and all gates are working and installed
  10. Screen door, air con casing and grills are tight and properly located

Last October 12, 2005 at 11:00 pm, I was driving a car on Acacia St. Sampaloc, Manila and suddenly involved a collision with a car. My car has been damaged such the headlights are broken and the bumper has come out. I immediately took pictures of the damaged portions as a proof to support my argument. The estimated cost of repairs is approximately Php25,000.00.

I already attached my insurance paper and it says that I am eligible for an insurance claim. I have attached the copy of the estimates as well as the the picture of damages.
