Good day!

This is to inform that may car has been damaged and I would like to claim the insurance amount for it.

Last October 12, 2005 at 11:00 pm, I was driving a car on Acacia St. Sampaloc, Manila and suddenly involved a collision with a car. The estimated cost of repairs is approximately Php25,000.00.

I already attached my insurance paper and it says that I am eligible for an insurance claim. I have attached the copy of the estimates as well as the the picture of damages.
