May I request your good office to have a maternity leave to start approximately October 1 to November 30, 2000. My doctor advises me rest as soon as possible since I am scheduled to undergo a C section on October 8, 2000.
My handled account will be distributed to our sales person while I am on leave. I will instruct who ever assigned to my current work while I am away from work. I want to ensure that my job responsibilities will be managed during that period.
