Bank Complaint Letter – Delay Issue of Cheque Book

Dear Mr. Samaniego:

This is to bring to your kind notice that I had applied for a cheque book/pass book about 15 days ago.  The clerk told me to collect my cheque book on the stipulated date, the clerk told me to collect my cheque book a day later, I have three rounds of the bank on different days but was always told to come later. It causes me troublesome because of your unprecedented lack of service.

I request you to issue me a fresh cheque book as soon as possible because I need it for my employment.
