Decline Letter for Covid Vaccine

July 7, 2021

To whom it may concern:

I am choosing to decline COVID-19 vaccination right now. I am truly aware the effects of my decision but I do believe that this is wise choice since I have allergies to vaccine contents.

Based on my knowledge the vaccine is on trial stage so there is no guarantee that it will be safe to use especially for those who have weakened immune system.

I hope for your understanding regarding this matter.

Sincerely yours,

Patricia A. Vienne

Ronnie M. Liers
Consultant
JKL Enterprise Inc.

Dear Mr. Liers:

I am grateful of working with you for the past two (2) years but we have different point of views. Our company will suffer in the future if we have different decisions so it is better to end our business partnership.

I already talk to my personal lawyer regarding our issues and advised me to hire an auditor to audit our business assets and liabilities to ensure that we have equal sharing of profits and business properties. I would like to set an appointment regarding this matter

A sincere thanks for all the knowledge you have share with me. I hope that we will remain a good friend after dissolving our business partnership and don’t take it personally.

I hope for your reply as soon as possible to settle everything.

Thanks, and more power!

Sincerely yours,
Lian Leen

Request for Donation Letter

March 15, 2019

Dear Sir/Madam,
Greeting for ABC Learning School in her continuing Service to Education.

This center was conceptualized and built at the bedrock of service – service to God, Country and Fellowmen. With the remarkable achievement of our teachers and pupils it seems to close her down due to Pandemic Covid 19. Parent’s failed to pay their obligations because of the outbreak of Covid 19 before the last payment schedule. Thus, we decided to embark on a GRANT-IN-AID program.
It is said the “CHARITY is the PERPECTION of ALL LOVE”. Our GRANT-IN-AID program hopes to achieve the following objectives:

  1. To provide each student a learning strategy to cope up with online learning.
  2. To help parent to be able to adapt learning modules that will help their children to become independent in answering their modules and weekly assignment
  3. To sustain quality education despite Covid 19 pandemic

Together let our Golden Harvest in life be our legacy of Faith and Love to all who will pass the portals of this institution.
Thanks and more power!

Volcanic Activity Notification Letter

January 28, 2021

To whom it may concern:

This is to notify the public and concerned authorities of recent earthquake activity in Pinatubo Volcano.

Since 20 January 2021, the Pinatubo Volcano Network (PVN) and the Philippine Seismic Network (PSN) have detected a total of 826 imperceptible earthquakes east-northeast of Pinatubo Volcano in the vicinities of the City of Mabalacat, Pampanga. The strongest of these earthquakes occurred on 25 January at depths of 15-28 kilometers and ranged between ML1.0 and ML2.5 in magnitudes. Based on the general distribution of located events, these earthquakes were generated along a segment of the Sacobia Lineament, a fault that was last active during the post-eruption period of 1991.

This development serves to remind us once again that several faults occur around the volcano and that these can generate earthquake activity that can be sometimes imperceptible and harmless but that can also sometimes be damaging. On 22 April 2019, a M6.1 earthquake on the southern foot slopes of Pinatubo Volcano shook the Central Luzon region and caused fatalities and substantial damages to buildings, roads and other structures.

DOST-PHIVOLCS would like to assure the public that Pinatubo Volcano remains at Alert Level 0 and remains in quiescent condition. Although there is currently no threat from the above recent earthquake activity, communities and local government units surrounding the volcano, nonetheless, have to be reminded to be always prepared for both earthquake and volcanic hazards and to build on the lessons of what they have experienced in the past years. DOST-

PHIVOLCS continues to strengthen its monitoring of the volcano with ongoing upgrades to the PVN and upcoming geochemical surveys of the Pinatubo crater lake. DOST-PHIVOLCS is closely monitoring the volcano’s condition and any new development will be relayed to all concerned.
