I would like to inform your good office that I am authorizing my sister, Evelyn D. Ramos for claiming my Pag-ibig ID. I am not able to go to your office because of my work.
Ms. Ramos will bring necessary document to claim my Pag-ibig ID. She will also bring her two (2) identification card for your reference and verification.
